It’s 1963 and Poplar is covered by a thick blanket of snow as the nuns and nurses of Nonnatus House face the challenge of the coldest winter for three hundred years. Battling against snow, ice, power cuts and frozen pipes, the midwives brave the elements to provide the best possible care for their patients. Valerie helps a young couple newly arrived in London, who experience a traumatic birth, and Sister Julienne strives to reunite a family torn apart by a tormenting father.

The ‘Big Freeze’ continues with scheduled power cuts and more blizzards. Sister Julienne recruits a new midwife, Lucille Anderson. Owing to the snow, Lucille is two days late and her start is bumpy when she falls ill and is called out from her sick bed to assist Trixie with a breech birth. Meanwhile, Nurse Crane and Dr. Turner care for an elderly Jewish woman suffering from cancer who is facing an eviction from her her home. Shelagh considers hiring an au pair and Sister Winifred is nervous about her approaching driver’s test.

Lucille falls victim to prejudice and racism when a new mother unexpectedly falls ill. Nonnatus House must rally behind Lucille to restore her confidence in the community. Sister Winifred makes a case for fathers to be more involved in their wives’ pregnancies but finds the men of Poplar may need more persuading. A romantic dinner doesn’t go quite as planned for Trixie and Christopher, and Shelagh gets a little more than she bargained for with her new Au Pair.

Nurse Crane and Trixie get involved with the Lunt family when the mother’s behaviour causes some concern and the rest of the family are struggling to cope. Dr. Turner refers Doreen to a Neurologist and his diagnosis brings devastating news for the whole family. An admission from causes a strain on his relationship with Trixie. Violet and Fred decide to organize the very first Poplar Beauty Contest to raise money for charity. Violet invites Lucille to get involved and she throws herself into management mode. Valerie and Magda sign up to compete and Magda asks Valerie to keep a secret with devastating consequences.

Sister Julienne is drawn into the heart of a different culture when she is called upon to help a Pakistani family struggling to come to terms with childlessness and the husband marrying a second wife. Elsewhere, Trixie struggles to cope and looks for comfort in the bottom of the bottle. But it’s not long before she can no longer conceal her struggle with sobriety. Sister Julienne and Dr Turner are convinced Sister Monica Joan needs cataract surgery, but she’s definitely not convinced. At Clinic, Lucille meets a woman with Pica but this is no ordinary craving.

Trixie makes a decision about her future. There is concern when a rumour spreads that a Nigerian seaman is hiding somewhere in Poplar with smallpox. Nurse Crane and Dr. Turner race to find him to prevent an outbreak. She’s unsure how she’ll cope with two children, one on the way and a business to run. Valerie is busy coaching Lucille how to lead the Health and Relationship classes at the youth club but the classes get disrupted by a furious mother. Why? At Nonnatus House, Sister Monica Joan prepares to leave for her cataract surgery. How will it go at the hospital?

Barbara falls ill, leaving Tom stricken and Nurse Crane struggling to cope. Sister Julienne returns to the roster to ease Nurse Crane’s workload and even Shelagh puts on the red cardigan once more. Meanwhile, Dr Turner has been asked to cover for a colleague at Wadelock House, a remand home for boys. There Dr Turner meets Michael Sumpter, a 16-year-old boy who’s showing signs of depression, awaiting trial and expecting a baby with his wife, Alison. The Turners are determined to help the young couple in any way they can. Elsewhere, Lucille cares for fellow Jamaican Alecia Palmer and discovers that her husband is a Pastor and they hold church services in their front room. Alecia invites Lucille to join them but she isn’t sure what path to take.
